Chief purpose of this position in support of above mission or goal

This position is for a full-time 12-month term (non-tenure-track) Clinical Faculty member in the Department of Family and Community Health. The chief purpose of the position is to assist with project development and service provision for the community based Health and Wellness Program for Older Adults. Primary functions include 1) assisting with precepting of interprofessional student teams including BSN and Master’s nursing students, social work, pharmacy and medical students at the program site; 2) providing care coordination to patients receiving care within the program and 3) assist with program development and refinement and preparation of reports, etc. To effectively implement this role, the incumbent must be highly skilled in communication and organizational skills with clinical knowledge and skills related to chronic disease management for vulnerable community dwelling populations. Experience with interprofessional team collaboration is desired but not required.

Position Responsibilities

• Teach and precept in clinical venues related to care of adult patients in the Health and Wellness community based program
• Assist with development of structures, processes, policies, procedures and reporting
• Enhance team experiences related to interprofessional competencies in undergraduate and graduate education

Research (Scholarship)
• Demonstrate evidence of building scholarship
• Demonstrate efforts to improve clinical practice at the project site

• Participate with professional organizations at the local or state level
• Participate in VCU School of Nursing service committees as required by faculty

• Assist with leading the development of an innovative clinical experience focused on community dwelling older adults that are undeserved
• Provide clinical care as directed for a population of older adults within the program requiring primary and transitional care
• Act as coordinator for clinical service delivery at the program sites

Required Qualifications

Preparation as Master’s prepared family or adult- gerontological nurse practitioner. Evidence of successful board licensure in specialty practice. Experience in working with chronically ill complex older adult populations

Preferred Qualifications

1 or more years experience in working with complex chronically ill adults
